Scrip Order Form
The CCHS Scrip Program offers a tuition incentive to participating families at 50% of the Scrip card discount amount (the remaining 50% will go to the CCHS Tuition Assistance Program). To order, please complete the form below and mail it with payment to CCHS c/o Finance Office. All orders will be placed every Monday morning by 10 a.m. and will be ready for pick-up on Friday.
